Medium depends on the last the shoe is made on. Usually it is a between a c & d. We used to refer to Medium as C-D.
It really depends on the last. Some shoes have a higher arch, some shoes have a deeper toe box. These things also effect the fit. Some laces are what is called a bal cut, & some are a blutcher cut. This also will effect the width feeling. In reality a Medium is a D, but it may be a bit tighter. This shoe appears to be made on "The Sheffield Last", but I'm not sure. If it's on The Sheffield Last it will fit fuller than many more casual shoes or shoes on different lasts.
