It does not come with a camisole. It's mesh, but it's not a really loosely knit mesh, so the only place it's see-through is where it's stretched, so for me, that's across the chest. I didn't want my bra to show at all, so I put on a basic cami underneath that wouldn't add a lot of heat. If you want to wear a t-shirt or long-sleeve shirt underneath, I may not go a whole size up, but if you're on the border of two sizes, go with the higher one. If you don't plan to wear more than a tank top or camisole underneath, I would say go normal size. It's very soft and warm, not typically what you'd think when you hear "mesh." The only mesh thing about it is that you could potentially see your bra through it if you get a size that hugs your chest.
